Disparities logic model - 6/15/07

Q: Michigan is in the process of revising and updating the logic models for each of the program goal areas.  Does anyone have a tobacco related health disparities program logic model that they'd be willing to share?

A:

  1. Indiana: Attached are the following documents that were used to promote/support the implementation of the disparities plan in Indiana:
     
    1. The agenda for the stakeholders meeting that Smokefree Indiana held in order to promote the plan to the state and local public health leaders.
    2. A disparities grid that identified organizations and their action plan activities that would fall under the disparities plan.
    3. A logic model for Indiana’s disparities plan.
    4. An agreement form that we had requested all the attendees of the stakeholders meeting to sign in order to commit their organization to address disparities.
       
  2. Kentucky: Kentucky’s logic model for disparate populations related to tobacco use is available for viewing; it needs to be updated.
     
  3. Nevada: Nevada’s logic model is available for viewing.
     
  4. Oregon: Oregon does not have a tobacco related health disparities logic model. But, we have been working with partners in the American Indian (tribal and urban), African American, Asian & Pacific Islander, Latino, and LGBTQ community (populations experiencing disparities) to develop logic models for tobacco prevention policy objectives and coalition building. This has been facilitated by external evaluation consultants using community-based collaborative research methodologies. We should be completing these projects by the end of the summer.
     
  5. Mississippi: Mississippi’s the tobacco-related disparities logic model that was approved by the Mississippi Department of Health in January 2007. We received the tobacco-related disparities grant from CDC this year, and we are currently developing a tobacco-related disparities strategic plan for our state.
     
  6. Ohio: A logic model focusing on disparities is located on page 19 of the Action Plan.
     
  7. West Virginia: West Virginia does not have a disparities logic model. See our DTP Minority Statement.
