Prescription pad promotional materials for Quitlines – 6/23/05

Q:

We are interested in using "prescription pad" promotional materials for our state Quit Line. I know a few states had mentioned at the CDC meeting that they were using them. We are hoping to have them sent out statewide with our provider material updates, have them in local/district health departments, and provide them to our local tobacco coordinators and coalitions so they can present them to doctor's offices and pharmacies. Would anyone be willing to share a sample of the prescription pad they have developed and are using?

A:

  1. Arkansas: Our quitline vendor created prescription pads to send out with provider toolkits. They can be viewed at http://sosquitline.pnms.com/.

  2. Oklahoma: see PDF - English, see PDF - Spanish

  3. South Carolina: SC produced a "faux prescription pad" for use by healthcare providers as a handy tool to help them advise their patients to quit and call the quitline number. We are using the Rx pads the very same way this state is considering: distributed to medical practices, hospitals, and other healthcare settings, as well as to our public health districts/health departments and local tobacco coalitions. The pads are intended to be used by healthcare providers who write scripts, e.g., physicians, physician assistants, nurse practitioners, or work with providers who do. A sample is attached. The pad is approximately 4.25" x 5.5" and has 50 tear-off sheets with English on the front side and Spanish on the back. See attached.
