Program Evaluation Plans - 8/3/07

Q: South Dakota is in the process of developing a written evaluation plan for its program. Do other states have examples (electronic, if possible) of their evaluation plans specific to developing a program evaluation plan that they would be willing to share?

1. Indiana - Attached is the PDF version of our current evaluation plan:

2. North Carolina – See the attached document; it is being updated but the format and layout might prove helpful:

3. North Dakota – Attached is the proposal that was submitted for North Dakota’s program evaluation.  The focus groups were very helpful to us as a program as we are taking the opportunities of improvement to assist with some changes at the state level.
