Statewide program evaluation plans – 3/11/11

Q: Pennsylvania is developing a written evaluation plan for its statewide tobacco program. Do other states have examples (electronic, if possible) of the evaluation plans, or other specifics relating to the development of a program evaluation plan that they would be willing to share?

1. Louisiana: Click to view Louisiana’s evaluation plan for 2010-2011.
    
2. Kentucky: Kentucky was required (as all grantees) by the ARRA funding to develop an evaluation plan for Components I & III.  The Prevention Research Center at the University of Kentucky provided assistance as a contractor.  Erika Fulmer, OSH Epidemiologist, also provided technical assistance.  Attached are the plans submitted for review by CDC:
