Communities of Excellence Key Indicator Assessment Forms – 2/2/07

Q:

Has your organization made revisions or upgrades to the Communities of Excellence Key Indicator Assessment Forms (hyperlink to attached PDF) that have led to increases in quantitative information/data collection rather than the current format that is based on the collective wisdom of coalition members?

A:

  1. Alabama – Alabama did the first version of Communities of Excellence, so we do not have any information regarding your newer version.
     
  2. California – The California Department of Health Services has updated its Communities of Excellence Guides. See the links below for the new documents and for the 2003 evaluation. Local health departments in California submit their data online so we are able to manipulate the data to create statewide reports that identify change over time and the amount of funds being spent on various indicators.  
  3. West Virginia - We used Communities of Excellence extensively here in West Virginia a few years ago, and it was instrumental in our initial education for our facilitation of local community coalitions.  We have not been using Communities of Excellence for over two years now.
     
  4. Wyoming - Wyoming used the Communities of Excellence as one of the needs assessments projects for the Tobacco-free Wyoming Community Program. The assessment is updated periodically (2 or 3 years) and used to gauge change over time. The tools include:
