Sample contracts for the Youth Tobacco Survey (YTS) – 12/21/11

Q: We are looking to do a contract for the 2012 Youth Tobacco Survey (YTS) and would like to see if any other states have examples of contracts in place so we can see what is being done and what is working already. If you have worked with / are working with a contractor to administer the YTS, please share a sample contract.

A:

  1. Alabama: The Alabama program does its own YTS data collection.Our Health Department staff (Area Tobacco Control Coordinators) do the data collection for YTS, so it's all in-house, except CDC does the data cleaning and draws the sample.
     
  2. Delaware: Delaware’s 2012 YTS contract is available from the Tobacco Control Network staff upon request; please contact us at tcn@astho.org.
     
  3. New Hampshire: The New Hampshire Tobacco Prevention and Control Program conducts the Youth Tobacco Survey collaboratively with the CDC’s Office on Smoking and Health(OSH) and its contractor, RTI International.  The actual coordination of the survey is done by program staff and selected school staff administers the questionnaire.  There are no contracts in place at this time.
     
  4. Virginia: The contract from the Virginia Foundation for Healthy Youth is available from the TCN upon request; please contact us at tcn@astho.org.
     
  5. West Virginia: In West Virginia, we do this survey in collaboration with the WV State Board of Education's Office of Healthy Schools and CDC/OSH Epidemiology staff. We never actually did a ‘formal’ MOU for our “Coordinated School & Public Health Partnership.” It is actually a contract (attached here).
     
    Additionally, attached are last year’s (2011) and this year’s  (2012) summaries that we have used to propose the collaborative tobacco-related work we are doing with our WV Department of Education over the past couple years.
